We coordinate quality healthcare for qualified, low-income, uninsured Collier County Florida residents ages 19 to 64.

Medical Services

Our network of physicians, hospitals, and other healthcare facilities and providers offer free medical care for qualified residents including consultations, procedures, surgeries and laboratory services through an integrated system.

Prescriptions, Pharmacy, Laboratory, & Imaging Service

Collier County residents may also be qualified to receive these free services through PLAN’s program.

Case Management

By helping our participants navigate the local healthcare system, we help them receive the medical care they need.

Mission

To make a significant difference in the health of our community by providing a safety net and increasing access to quality healthcare for all low-income, uninsured adult residents of Collier County.

PLAN is fulfilling its mission despite the implementation of the Affordable Care Act (ACA). There are many variables, both federally, statewide and locally, that have impacted the implementation of the Act and as a result of these, we continue to operate as we have done in the past.

The reality is that there will still be people in Collier County who will not be able to afford the premiums, choose not to participate, or cannot gain access to providers. PLAN recognizes the potential effects of the ACA, and the staff and board are monitoring these effects as the law continues to evolve.

History

Collier County Medical Society (CCMS) physicians were providing free medical services to its community and wanted a program that would:

  • Address the gaps in access to healthcare in its community;
  • Provide coordinated services;
  • Embrace the Florida Department of Health’s Volunteer Healthcare Providers Act which provides sovereign immunity for pro bono care.

In 2003, CCMS launched The Collier We Care initiative based on the Project Access Buncombe County Medical Society model that presents an integrated, organized model of volunteer medical care.

In the summer of 2004, Collier We Care received its 501(c)(3) status and later became Physician Led Access Network (PLAN) of Collier County in January 2005.
